Job Description:



  • Patient Care:

    • Administer medications and treatments as prescribed by the healthcare provider.

    • Monitor patients' vital signs and report any changes in condition to the RN or physician.

    • Assist in developing and implementing individualized care plans for residents.

    • Provide basic nursing care, including wound care, catheter care, and assisting with daily living activities.

    • Ensure patient comfort and safety by responding promptly to call lights and addressing patient needs.




  • Documentation and Communication:

    • Accurately document patient care, observations, and interventions in the medical record.

    • Maintain open and effective communication with patients, families, and the healthcare team.

    • Participate in shift handoffs to ensure continuity of care.




  • Team Collaboration:

    • Work collaboratively with RNs, CNAs, and other healthcare professionals to deliver comprehensive care.

    • Assist with the training and orientation of new staff members as needed.

    • Participate in care conferences and contribute to the interdisciplinary team's decision-making process.




  • Compliance and Safety:

    • Adhere to all facility policies, procedures, and regulations, including those related to infection control and resident rights.

    • Ensure proper use of medical equipment and report any malfunctions to appropriate personnel.

    • Maintain a clean, safe, and organized work environment.



Qualifications:


  • Education: Graduate of an accredited LVN program.


  • Licensure: Current and valid LVN license in the state of California.


  • Experience: Prior experience in a skilled nursing or post-acute care facility is preferred.


  • Certifications: BLS (Basic Life Support) certification required.


  • Skills:

    • Strong clinical and critical thinking skills.

    • Excellent communication and interpersonal skills.

    • Ability to work effectively in a fast-paced environment.

    • Proficiency in electronic medical records (EMR) systems is a plus.



Physical Requirements:

  • Ability to lift and move patients as required.

  • Prolonged periods of standing and walking.

  • Ability to perform physically demanding tasks, such as repositioning patients.
