About the Kidney Transplant APP Role and Transplant Dept



The Advanced Practice Provider (APP) (Nurse Practitioner/Physician Assistant) reports organizationally to the Abdominal Transplant Program Administrative, Nursing, and Clinical Directors. The APP is responsible for providing comprehensive care, assessing, diagnosing, and treating patients in a variety of settings, in collaboration with physicians and other healthcare professionals, including pharmacists, transplant nurse coordinators, social workers, dieticians, financial coordinators, medical assistants, research and administrative staff. The APP will also be responsible for providing education to patients and their families. The APP acts as the central point of contact and maintains open lines of communication between to the surgeons, physicians, and other members of the team. The APP also interacts with external stakeholders such as organ procurement organizations, transplant centers, insurance companies, pharmaceutical representatives, and community providers. The APP may supervise, or mentor other medical staff or students involved in the transplant program.


The APP functions in direct collaboration with the nephrologists and surgeons assigned to the Transplant Center. They initiate, facilitate, and develop interdisciplinary collaboration and participate in all aspects of the post-kidney transplant patient's care, providing and maintaining comprehensive documentation in the patient medical record. In collaboration with patients and families, she/he designs a plan of care based on an assessment of physical, emotional, and social needs with an understanding of the pre- and post-transplant protocols. The APP is also involved in some aspects of pre-transplant care and participates on committees and task forces that relate to organ transplantation. The APP also participates in our MDC (multidisciplinary clinics) for pre kidney transplant evaluation, providing a comprehensive history and physical on potential kidney transplant recipients, provide education about the transplant process, and participate in selection committee meetings.

Qualifications

Qualifications for a Kidney Transplant APP:




Skills:



  • Ability to comprehensively assess pre-transplant patients for their appropriateness for kidney transplantation.

  • Ability to manage post-transplant patients, including monitoring for signs of rejection and infection.

  • Proficiency in managing immunosuppressive therapies and recognizing transplant-related complications.

  • Exceptional time management and organizational skills providing follow-up care for transplant patients, including monitoring lab results, and adjusting treatment plans as needed.

  • Knowledge of the principles and practices of nursing, transplantation, organ donation and related fields.

  • Ability to assess, plan, implement and evaluate patient care in a holistic manner.

  • Ability to communicate effectively with compassion, empathy and respect to diverse populations using verbal, written and electronic methods.

  • Ability to work collaboratively and independently within a multidisciplinary team.

  • Ability to use critical thinking and problem-solving skills to prioritize, organize and manage multiple complex clinical and logistical issues in a fast-paced environment.

  • Ability to use computer software applications such as electronic health records, databases, and spreadsheets.

  • Knowledgeable and compliant in Transplant hospital policy and procedures, Transplant State and Federal regulatory requirements, UNOS and CMS regulations, the Joint Commission, and Health Insurance Portability and Accountability Act of 1996 (HIPAA).


Experience:



  • Previous critical care or medical/surgical experience

  • Experience in transplant, specifically in kidney transplantation or nephrology, with demonstrated clinical competence and professional accountability preferred.

  • Experience with EPIC preferred.


Education/Degree



  • Master's degree in nursing required.

  • Minimum of 6 years of NP or PA experience.


Licensure, Certifications, or Registration

  • Current licensure as a Nurse Practitioner or Physician Assistant in the Commonwealth of Massachusetts. Current DEA license (if NP)

  • Certified Clinical Transplant Nurse (CCTN) preferred.
