Job Description:

Patient Care:
- Administer medications and treatments as prescribed by the healthcare provider.
- Monitor patients' vital signs and report any changes in condition to the RN or physician.
- Assist in developing and implementing individualized care plans for residents.
- Provide basic nursing care, including wound care, catheter care, and assisting with daily living activities.
- Ensure patient comfort and safety by responding promptly to call lights and addressing patient needs.

Documentation and Communication:
- Accurately document patient care, observations, and interventions in the medical record.
- Maintain open and effective communication with patients, families, and the healthcare team.
- Participate in shift handoffs to ensure continuity of care.

Team Collaboration:
- Work collaboratively with RNs, CNAs, and other healthcare professionals to deliver comprehensive care.
- Assist with the training and orientation of new staff members as needed.
- Participate in care conferences and contribute to the interdisciplinary team's decision-making process.

Compliance and Safety:
- Adhere to all facility policies, procedures, and regulations, including those related to infection control and resident rights.
- Ensure proper use of medical equipment and report any malfunctions to appropriate personnel.
- Maintain a clean, safe, and organized work environment.
Qualifications:

Education: Graduate of an accredited LVN program.

Licensure: Current and valid LVN license in the state of California.

Experience: Prior experience in a skilled nursing or post-acute care facility is preferred.

Certifications: BLS (Basic Life Support) certification required.

- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Proficiency in electronic medical records (EMR) systems is a plus.
Physical Requirements:
- Ability to lift and move patients as required.

- Ability to perform physically demanding tasks, such as repositioning patients.
