Key Responsibilities
Conduct Assessments: Perform comprehensive biopsychosocial evaluations to assess the needs of clients struggling with behavioral health and SUD challenges.
Individualized Care Plans: Design, implement, and regularly update personalized treatment plans to ensure effective client care.
Counseling Services: Provide compassionate, client-centered therapy, including individual, group, and family counseling sessions.
Collaborative Care: Work closely with a network of service providers and agencies to deliver holistic, coordinated care.
Discharge & Transition: Facilitate the transition planning process, ensuring clients are fully supported as they move through their care journey.
Crisis Intervention: Offer immediate support during crises, maintaining a safe and therapeutic environment.
Documentation: Ensure timely and accurate completion of all clinical documentation, meeting organizational and regulatory requirements.
Team Participation: Engage in case reviews, clinical presentations, and utilization reviews, contributing to a collaborative team dynamic.
Qualifications
Required Licenses: LCSW-C, LCPC, or LCADC. Supervised positions are available for LGPC and LGADC candidates.
Experience: At least one year of experience in behavioral health or SUD treatment settings is required.
Preferred: Experience working in Intensive Outpatient Programs (IOP) is highly valued.
Skills: Ability to manage multiple tasks in a fast-paced environment with a strong commitment to client care.
