JOB SUMMARY

The Upward Mobility Mentor/ Coach will ensure that clients have access to comprehensive, community-based services, and that all requirements from referral sources are met. Focus on engaging clients from the time of the referral through the entire course of the client’s treatment services.

KEY RESPONSIBILITIES

  • Provides linkage and navigation assistance for clients and their families/significant others so they can access all relevant community services.
  • Helps clients and their families identify and access other Village clinical services/non-clinical services and/or other community resources. Screening will provide the basis for referrals, and navigation services will include advocacy and follow-up. Other Village services may include specialty treatment for children and adults.
  • Maintains and updates service data to ensure accurate reporting, when required.
  • Takes steps to ensure client engagement from referral through transfer/discharge from service, including initial outreach to clients upon receiving referrals.
  • Conducts baseline Intake on all clients upon entering the program.
  • Implements Service Plan with client on specific targeted goals to achieve while in program.
  • Conducts client surveys/feedback on program.
  • Implements service plan reviews with client every (6 months or as necessary).
  • Implements discharge service Plan and Client survey of program upon completion of program.
  • Provides comprehensive, targeted coaching & case management services to clients.
  • Completes all required documentation for client while in program (service plans, reviews, progress notes, required data).
  • Develops and maintains collaborative relationships with The Village Programs and other community agencies, organizations, and referral sources
  • Maintains effective, positive working relationships with referral sources.
  • Performs accurate and timely completion of required documentation and reports, including entering client level data into database.
  • Meets all standards for timeliness and accuracy in performing data and reporting functions, including communication with referral sources.
  • Comply’s with all agency policies and procedures, HIPAA regulations, Joint Commission standards and funding sources requirements.
  • Participate in individual/group/team supervision, clinical quality improvement and Quality Assurance activities; participates in Utilization Review.
  • Contribute to agency visibility and development of community linkages by representing the Village in community groups or committees, when directed by Supervisor.
  • Perform other duties and responsibilities as assigned by management

Experience and Skills

EDUCATION

High School diploma required, Bachelor’s Degree preferred

EXPERIENCE

  • Excellent oral and written skills and ability to communicate comfortably with children, parents, and both primary care and behavioral health professionals
  • Organized and able to work independently with minimal supervision.
  • Flexible and able to multi-task.
  • Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows environment. Familiarity with specific applications used by The Village and its referral sources is a plus.
  • Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred
