Our organization is seeking qualifiedHospice Registered Nurse Case Manager (RN) or Licensed Practical Nurse (LPN)to work with our geriatric population in their own homes. Responsibilities include comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. The nurse will work as a liaison between physicians, families and the administrative nursing team to ensure a continuum of care.
This position requires weekend availability possibility, Hours during the week 5:00 PM - 8:30 AM
