Clarity Wellness Community is a progressive not-for-profit organization providing mental health services in Allegany and Wyoming counties. Clarity's mission is to empower people with care, connections and support for a lifetime of choice. We are currently seeking a full-time HBCI Provider to support children and families on their path to wellness.

Home-Based Crisis Intervention Providers aim to empower and stabilize families by providing short-term (4-6 weeks), intensive support to avoid unnecessary psychiatric hospitalizations of children and youth ages 5-21 years. This is accomplished using evidence-based practices to assist youth and their families with safety planning, and obtaining needed medical, social, recreational, educational, financial, vocational, and other services through assessment, goal setting, progress monitoring, and collaboration with other professionals.

Responsibilities:

  • Schedule sessions at times that are convenient for families, and when problems are likely to occur, including evenings. Maintain a flexible workweek.
  • Schedule and complete client intake sessions within 48 hours of referral.
  • Participate in weekly group supervision and individual supervision
  • Participate in training and professional development opportunities.
  • Participate as a member of the SPOA committee.
  • Provide a range of clinical, concrete and advocacy services to family members that are consistent with the family's values, learning styles, lifestyle, circumstances and culture.
  • Assume responsibility for motivating families and employ motivation enhancement strategies.
  • Develop and implement safety plans for youth and families.
  • Provide de-escalation and support for youth and families during crisis situations.
  • Provide services primarily in client homes. Travel to client homes and other locations.
  • Collaborate with family members and other providers in developing intervention goals and service plans.
  • Utilize research-based cognitive/behavioral strategies to facilitate behavior change.
  • Assess goal progress and families' ongoing needs.
  • Utilize the agency's Electronic Health Record (EHR) system for all clinical documentation.
  • Complete and submit all clinical and agency paperwork in a timely and accurate manner.
  • Work with supervisor to complete professional development plans and performance reviews.
  • Refer to or link client to other community-based service providers as an alternative to inpatient evaluations and/or hospitalization, as indicated by the family and needs assessment.
  • Complete a comprehensive assessment that will identify client needs, risks, supports, cultural information, and religious information. This will provide the groundwork for work with the individual.
  • Develop an individualized treatment plan with the youth and their family within 7 days of admission to the program. The treatment planning process is recovery-oriented, building upon the client's strengths to help meet their needs.
  • Discuss relevant cultural, religious, ethnic, racial, developmental, age, social, sexual orientation, geographic, and other factors that influence and play a role in the client's life.
  • Be aware of and adhere to HIPAA regulations and agency's confidentiality and record keeping policies and procedures.
  • Perform all necessary discharge and termination functions for individual clients as needed within one week of discharge/termination.

Qualifications: A Master's degree in Psychology, Social Work, Mental Health or related field required. Applicants are expected to be licensed by the New York State Education Department and operate within the practitioner's scope of practice as defined in NYS law. Licensed mental health providers (LMHP's) include: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, and Licensed Creative Arts Therapist. Unlicensed MA staff with at least one year of relevant experience in community-based mental health or case management may be considered.

  • Schedule: HBCI Providers are expected to be flexible and schedule appointment times that are conducive to individuals and families. This may include appointments after school hours and into the evening. HBCI Providers take turns being on-call where they are expected to respond to client emergencies 24 hours per day seven days a week (on-call is rotated with other staff)
