Overview

The Transplant Services Coordinator is responsible for initiating, integrating and coordinating all renal transplant activities in Legacy Health System, and overseeing the operational activities of the dedicated transplant unit at Legacy Good Samaritan Hospital and Medical Center. The Coordinator works with employees, management, and physicians to develop systems and procedures necessary for all transplant activities, and provides training, information and support required by employees to solve problems and to accomplish the objectives of the Kidney Transplant Center.



Responsibilities

Coordinates the process of service delivery for designated area, including patient screening, patient flow, case management and follow-up for renal transplantation.

Utilizes United Network for Organ Sharing (UNOS) guidelines and other sources to determine program standards.

Works with key staff and physicians to develop a coordinated, integrated renal transplantation program within Legacy Health System.

Performs pre-transplant assessment with nephrologist and provides education to potential transplant recipients and their families and living related donors.

  • Assesses barriers to learning
  • Provides appropriate education based on assessment
  • Documents education in the chart
  • Revises educational tools as necessary to meet above needs

Coordinates the pre-transplant care plan for potential recipients and living related donors.

  • Facilitates the scheduling of medical testing with the appropriate testing facility
  • Schedules the tissue typing, monthly antibody screens, and cross match/compatibility testing
  • Develops a plan of care for the individual
  • Communicates information with medical team, dialysis unit, and patient

Coordinates admissions of transplant patients and donors if appropriate.

Living Donor Recipients:

  • Coordinates final tissue typing and pre-admission registration
  • Coordinates hospital admission on morning of transplant surgery

Cadaver Transplants:

  • Follows admission of cadaver transplant procedure
  • Coordinates admission of transplant recipient

Performs rounds on newly transplanted patients.

  • Reviews and records labs, vital signs, medications, and input/output on transplant flow sheet
  • Recognizes and communicates abnormal lab and test results to medical team
  • Communicates patient and family concerns to the medical care team
  • Performs post-transplant education and documents on education flow sheet

Coordinates discharge planning.

  • Coordinates outpatient clinic appointments and lab draw
  • Coordinates medication prescriptions

Coordinates patient care post transplant.

  • Assesses and cares for patient in transplant clinic
  • Performs medication list update
  • Communicates identified problems to appropriate medical team member
  • Documents on progress notes appropriate information for long term follow-up
  • Assesses and records post-transplant labs, communicating abnormals to medical team
  • Performs ongoing coordination of prescription medications assuring patient has appropriate medications
  • Communicates changes in medication to patient
  • Communicates changes in treatment regime to patient

Coordinates outpatient procedures.

  • Schedules ultrasound with or without biopsy
  • Schedules treatments in Day Treatment Unit
  • Assists with procedures performed in clinic

Performs education of post-transplant care and medication.

  • Assesses barriers to learning
  • Provides appropriate education regarding care and medications to transplant recipient
  • Documents education activities on educational flow sheet

Coordinates admissions of post-transplant recipients to the hospital.

  • Arranges bed for patient through bed control
  • Obtains post-transplant orders and ensures that the appropriate nursing unit(s) receives necessary patient reports
  • Performs rounds on hospitalized patients to monitor labs and medications and assists with problem resolution

Coordinates discharge planning.

  • Coordinates outpatient appointments with medical care team
  • Coordinates follow-up outpatient lab


Qualifications

Education:

Academic degree in nursing (BSN or MSN/MN) required; MSN/MN preferred. Internal candidates may be considered with verification of a BSN or MSN/MN in progress

Experience:

Minimum of five years work experience related to area of responsibility required.

Knowledge of regulatory requirements and relationships with related organizations including Centers for Medicare & Medicaid Services (CMS) and United Network for Organ Sharing (UNOS) required.

Management experience or experience in developing kidney transplant programs preferred.

Skills:

Thorough knowledge of operations, including staffing and scheduling, budget management, and workflow planning.

Ability to manage a broad span of control through implementation of a self-directed team approach.

Strong communication and leadership skills, and a willingness to lead by example.
