The Transplant Services Coordinator is responsible for initiating, integrating and coordinating all renal transplant activities in Legacy Health System, and overseeing the operational activities of the dedicated transplant unit at Legacy Good Samaritan Hospital and Medical Center. The Coordinator works with employees, management, and physicians to develop systems and procedures necessary for all transplant activities, and provides training, information and support required by employees to solve problems and to accomplish the objectives of the Kidney Transplant Center.
Coordinates the process of service delivery for designated area, including patient screening, patient flow, case management and follow-up for renal transplantation.
Utilizes United Network for Organ Sharing (UNOS) guidelines and other sources to determine program standards.
Works with key staff and physicians to develop a coordinated, integrated renal transplantation program within Legacy Health System.
Performs pre-transplant assessment with nephrologist and provides education to potential transplant recipients and their families and living related donors.
Coordinates the pre-transplant care plan for potential recipients and living related donors.
Coordinates admissions of transplant patients and donors if appropriate.
Living Donor Recipients:
Performs rounds on newly transplanted patients.
Coordinates discharge planning.
Coordinates patient care post transplant.
Coordinates outpatient procedures.
Performs education of post-transplant care and medication.
Coordinates admissions of post-transplant recipients to the hospital.
Coordinates discharge planning.
Education:
Academic degree in nursing (BSN or MSN/MN) required; MSN/MN preferred. Internal candidates may be considered with verification of a BSN or MSN/MN in progress
Experience:
Minimum of five years work experience related to area of responsibility required.
Knowledge of regulatory requirements and relationships with related organizations including Centers for Medicare & Medicaid Services (CMS) and United Network for Organ Sharing (UNOS) required.
Management experience or experience in developing kidney transplant programs preferred.
Skills:
Thorough knowledge of operations, including staffing and scheduling, budget management, and workflow planning.
Ability to manage a broad span of control through implementation of a self-directed team approach.
Strong communication and leadership skills, and a willingness to lead by example.
