Position Summary:


The MDS Coordinator is responsible for the day-to-day coordination and oversight of all aspects of the RAI and Care Planning process in accordance with Federal and State regulations.


Reports to:


Directly reports to the Director of Nursing of the facility where providing Nursing services.


Supervises:


If applicable, supervises administratively in the building assigned all staff of his/her respective discipline. Supervision follows all accepted state and federal guidelines for the respective discipline.


Recommended Minimum Position Qualifications


Education:


Graduate of an approved LPN/RN program. RN preferred.


Professional Experience:


MDS Coordinator experience preferred.


Certification/Licensure:


Actively licensed as an LPN/RN in assigned state(s) of employment.


Exposure Risk:


The MDS Coordinator can be at high risk for exposure to blood and body fluids, infectious diseases air contaminants (including tobacco smoke) and hazardous chemicals.


MDS Coordinator Essential Duties and Responsibilities


1. Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers


2. Conduct or coordinate the interviewing of each resident for the resident's assessment.


3. Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.


4. Assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family members.


5. Assist the Facility Educator in developing any training activities needed concerning resident assessment/care plan skills.


6. Attend and participate in annual facility in-service training programs as scheduled.


7. Attend and participate in continuing education programs designed to help you keep abreast of changes in your profession, as well as to maintain your license on a current status.


8. Conduct training programs for appropriate staff on the completion and use of the MDS, RAPs and care planning.


9. Monitor the facility's QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.


10. Ensure that all assessments are completed and transmitted in a timely manner. Report problem areas to the administrator/Director of Nursing.


11. Ensure that a current copy of the MDS Instructors Manual is available to persons completing portions of the MDS.


12. Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations or such disclosure to the administrator.


13. Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.


14. Report any known or suspected unauthorized attempt to access facility's information system.


15. Act as a chairperson of the Interdisciplinary Team


16. Work with the interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident.


17. Serve on, participate in, and attend various other committees of the facility as required, and as appointed by the administrator.


18. Provide written and/or oral reports of the resident assessment/care plan functions as requested or directed.


19. Participate in functions involving discharge plans as may be necessary.


20. Evaluate and implement recommendations from established committees as they may pertain to the assessment and/or care plan functions of the facility.


21. Maintain an effective, friendly working relationship with health professionals, physicians, consultants, and governmental agencies that may be involved in the resident assessment/care plan functions of the facility.


22. Meet with and solicit advice from department supervisors concerning the resident assessment/care plan functions of the facility.


23. Report known or suspected incidents of fraud to the Administrator, i.e. relative to false billings, cost reports, kickbacks, etc.


24. Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.


25. Ensure that all residents are treated fairly, and with kindness, dignity and respect.


26. Ensure that resident interviews are conducted in private.


27. Ensure that all members of the assessment team and the interdisciplinary/medical and nursing team are knowledgeable of the resident's responsibilities and rights including the right to refuse treatment.


28. Review any complaints or grievances made by residents regarding the resident assessment or care plan.


29. Develop comprehensive assessments of the nursing needs of assigned residents, utilizing the forms required by current rules or regulations and facility policies.


30. Ensure that appropriate health professionals are involved in the assessment.


31. Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties for false certification.


32. Coordinate the development of a written plan of care for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.


33. Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.


34. Ensure that an initial resident assessment is completed within fourteen days of the resident's admission.


35. Ensure that a comprehensive care plan is completed within twenty-one days of the resident's admission.


36. Assist the nursing staff in encouraging the resident and his/her family to participate in the development and review of the resident's plan of care.


37. Assist the Director of Nursing services and relevant directors/supervisors of other departments in ensuring that all personnel involved in providing care to the resident are aware of the resident's care plan.


38. Ensure that quarterly and annual resident assessments and care plan reviews are made on a timely basis.


39. Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition.


40. Coordinate the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status.


41. Assist the Director of Nursing services and other relevant directors and supervisors in maintaining good rapport and morale between and within departments to ensure that a team effort is achieved in developing and implementing a comprehensive assessment and plan of care.


42. Inform all assessment team members of the requirements for accuracy and completion of the resident assessment (MDS)


43. Ensure that each portion of the assessment is signed and dated by the person completing that portion of the


MDS.


44. Sign and date the assessment instrument to certify its completion


Daily duties include but are not limited to the following:


1. Scheduling of MDS assessments


2. Completion of MDSs


3. Completion of RAPs


4. Complete/update Care plans


5. Utilize the Daily Care Plan Update sheet and work collaboratively with the nursing team to ensure implementation.


6. Attend daily meetings as assigned


7. Conduct Resident Care Conferences per facility schedule


8. Communicate with interdisciplinary team members


9. Track admissions and discharges


10. Attend morning meeting/clinical report as assigned


11. Computer Application/Data Entry


Weekly duties include but are not limited to the following:


1. Transmit MDS's to State


2. Attend weekly resident "At Risk" meetings


3. Work collaboratively with nursing team to properly utilize and implement the MDS Alert Sheet.


Monthly duties include but are not limited to the following:


1. Complete monthly calendar for MDS completion and Resident Care Conference 2. Attend and participate in the Quality Assessment and Assurance meeting


2. Attend and participate in the QI/QM investigation meeting.


3. Attend all monthly staff meetings


Quarterly


1.Attend facility communication meetings


2. Attend Regional MDS Coordinator meetings


3. Other duties as directed by the Director of Nursing/Administrator
