Job Summary: As a Patient Care Specialist, you will play a key role in providing exceptional care to patients in a behavioral health setting. You will collaborate with healthcare professionals to support the overall well-being of our patients and ensure they receive the highest standard of care.
This position is for night shift (2pm-10pm) with rotating on-call hours.
Essential Functions:
Patient Support: Assist patients with intake procedures, gather necessary information, and ensure a smooth transition into care services. Offer emotional support to patients and families.
Coordination of Care: Collaborate with clinical staff, case managers, and other healthcare providers to ensure comprehensive care plans are followed. Coordinate follow-up appointments, referrals, and communication between providers and patients.
Patient Education: Provide information on treatment plans, medications, and behavioral health services. Educate patients on managing their health conditions and treatment adherence.
Administrative Duties: Handle phone calls, schedule appointments, and manage patient records. Ensure proper documentation of patient interactions, progress, and updates in electronic health records (EHR).
Crisis Intervention: Assist patients in crisis situations by providing immediate support and contacting appropriate staff or emergency services as necessary.
Compliance & Confidentiality: Ensure all patient information is handled in compliance with HIPAA regulations and RiverValley Behavioral Health policies. Maintain strict confidentiality and professionalism.

Qualifications:
Education and Experience:
High school diploma or equivalent required; associate or bachelor’s degree in a healthcare-related field preferred.
Experience in a healthcare, mental health, or behavioral health setting is a plus.
Strong interpersonal skills, with the ability to communicate effectively and compassionately with patients, families, and healthcare professionals.
Ability to multitask, manage time efficiently, and handle stressful situations.
Knowledge of behavioral health and mental health conditions, and familiarity with medical terminology.
Proficiency with electronic health records (EHR) systems and general office software.
