The CHIP Coordinator will play a crucial role in guiding health councils through the County Health Assessment process. This position involves identifying health-related goals and priorities, as well as assisting in the creation of a three-year action plan to foster positive health outcomes in local communities. Position will be based out of the Upper Cumberland Regional Health Office in Cookeville, TN. This is a contract opportunity, on-site full-time, 40 hours per week. Travel within the region will be required regularly, on average 2-3 days per week.
Key Responsibilities:
Collaborate with local health councils to conduct comprehensive County Health Assessments
Facilitate discussions to identify community health priorities and establish actionable goals
Develop and assist in the implementation of a three-year Community Health Improvement Plan (CHIP) in alignment with local health needs
Engage with stakeholders, including community organizations, healthcare providers, and government agencies, to build partnerships and leverage resources
Monitor and evaluate the progress of health initiatives and make recommendations for adjustments as needed
Conduct regular travel within the Upper Cumberland region to meet with health councils and stakeholders (2-3 days per week)
Prepare reports and presentations to communicate findings, progress, and recommendations to various audiences
Minimum Qualifications:
Bachelor's degree in Public Health, Health Education, or a related field is required
Master of Public Health (MPH) or relevant public health experience is preferred but not required
Experience in collective impact initiatives and/or leading coalitions is highly desirable
Strong analytical, organizational, and communication skills
Ability to work collaboratively with diverse stakeholders and build effective partnerships
Proficiency in Microsoft Office Suite and experience with data analysis tools is a plus
Salary and Benefits:
Salary is based on education and experience, with an hourly pay range of $35.00 to $43.00 per hour.
