• Current California LVN license.


• Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting.


• Strong understanding of MDS 3.0, care planning, and regulatory requirements.


• Excellent organizational skills and attention to detail.


• Strong communication and teamwork skills, with the ability to work collaboratively with a multidisciplinary team.
