POSITION OVERVIEW:







PURPOSE:



The Jewish Board's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.



POSITION OVERVIEW:



The Coordinator of Central Intake and Assignment aids and facilitates intake and assignment processes for all Community Behavioral Health Clinic and CCBHC clients. The duties associated with this position include, but are not limited to, daily tracking of all clients in pre-admit, intake, assignment or re-assignment status. The Coordinator of Central Intake and Assignment is responsible for managing and enhancing the client experience throughout the intake and assignment process by ensuring each client's needs are handled timely. Additionally, this position is responsible for supporting program operations by working closely with clinics, One Call and CCBHC staff in ensuring intake, assignment and re-assignment operations are conducted efficiently.



KEY ESSENTIAL FUNCTIONS:




Tracking and Daily Monitoring



  • Tracking clients in pre-admit, intake, assignment or re-assignment status.

  • Working with program to ensure timely pre-admit discharges.

  • Tracking new hires to monitor for changes in clinic and CCBHC client capacity

  • Tracking terminations to identify clients needing reassignment

  • Tracking inactive clients and collaborating with program to ensure such clients are scheduled or discharged as appropriate.

  • Monitoring changes in caseloads to identify case assignment opportunities.

  • Track future Intake appointments to monitor for changes in client pipeline.

  • Weekly review of intake no shows and cancellations to identify clients that need a pre-admit discharge or to be rescheduled.

EDUCATIONAL / TRAINING REQUIRED:



  • Bachelor's degree in business, healthcare or related field.

  • LCSW Preferred, LMSW Required.


EXPERIENCE REQUIRED / LANGUAGE PREFERENCE (List what the desire to reach goals):



  • Minimum four years of working in a busy administrative and/or clinical role.

  • Proven track record of excellent organizational skills and keen attention to detail.




COMPUTER SKILLS REQUIRED:



  • Experience working with electronic medical records and proficient with Microsoft Office, particularly Excel.




VIVISUAL AND MANUAL DEXIERITY:



  • The candidate should be able to read documents for analytical purposes such as computer information, software, computer graphic design programs, etc.



  • Limited applications of manual dexterity and hand-eye coordination.


WORK ENVIRONMENT / PHYSICAL EFFORT:



  • Hybrid work setting. Position entails a combination of in person and remote work.

  • Travel required between program locations and other Jewish Board sites.
