Job Description:

SUMMARY

Provides crisis response services within the community on an on-call bases this includes assessing

individuals in acute mental health crisis for level of care needed. Crisis services are provided in the office and within the community and on-site of the crisis.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Crisis Responsibilities:

1. Provides crisis intervention services in person or by telephone.

2. Participates in a rotation on-call schedule with all other providers assigned to the team.

3. Responds to and provides in-person on-site assessment at the community location within 60

minutes.

4. Meets with the client, who is an individual and/or family in acute crisis. Explains the evaluation

process and gathers information from the client for the assessment and discusses the information

with consulting QMHP to determine level of care needed, inclusive of referral up to inpatient

psychiatric supports.

5. Additional time dedicated to crisis stabilization intervention and treatment.

6. Provides case management services that coordinate, support and advocate for client, psychoeducation and parent coaching.

7. Provides support to individuals after crisis. Outreaches to engage clients who have not followed

through with post crisis recommendations.

8. Assists families by connecting to resources. Assists with continued placement efforts as needed.

9. Maintains records in accordance with PCH policies and completes required outcomes protocols.

10. Participate in PCH meetings and in-services.

11. Other duties as assigned.

EDUCATION AND EXPERIENCE

Bachelors degree in Psychology, Social Work, or related field required. Related experience should

include work with adults, children and adolescents, crisis intervention, assessment, case management,

and home and community-based interventions. Ability to work as part of a team and collaborate with consumers, referrals sources, and other community providers is absolutely essential.

Demonstrated Competencies

• Ability to recognize signs and symptoms of mental illness and substance use crisis.
• Must possess the skills to engage individuals in a compassionate, nonjudgmental manner.
• Experience in De-escalation preferred.
• Ability to relate to a diverse population.
• Knowledge of community resources
• Excellent written and verbal communication skills
• Bi-lingual: Spanish-speaking, reading, writing skills preferred.

LANGUAGE

Bilingual (English/Spanish) skills preferred.

OTHER

Must maintain a safe driving record and have current valid Illinois Driver's License. Personal vehicle

needed for transportation to/from home and community-based interventions. Mileage for authorized work-related purposes is reimbursable.
