Job Summary:
A Case Manager supports this mission in your daily care, interventions, teaching and guidance of Andrew House clients.
This is a non-exempt position.
Essential Functions of Position:
Completes the psychosocial and determines with the client (and co-workers when appropriate) the individual treatment and aftercare plans
Responsible for client aftercare referrals (follow-up substance abuse treatment) and placement
Promotes ongoing participation of the client with decisions involving plans, goals and progress
Ensures the completions of psychosocial, admission MIS form, and treatment plan within first 24-48 hours after patient admission
Arranges aftercare interviews or other related appointments by telephone after obtaining appropriate releases to disclose necessary information
Coordinates treatment services with external treatment providers, outpatient counselors/therapists, opioid treatment, and other partners such as primary care, DCF, Courts
Coordinates discharge date with nurse and insurance coordinator
Completes discharge summary and discharge MIS form
Maintains relevant and timely counseling progress notes
Provides a level of counseling conducive to a stable medical and psychological state during detox/withdrawal. Arranges referral to the BEST Team when indicated
Leads educational/recovery client groups as assigned
Attends daily report with other unit disciplines and communicate clients' plans and progress as coordinated with team members
Assists in team effort in handling crisis-oriented emergencies
Documents all interventions in the counseling progress notes
Available for weekend and holiday coverage in fair and equitable distribution
Attends staff meetings, supervision, in-service conferences, periodic reviews and other relative meetings as determined by the clinical or program director
Participates as an active member of the multidisciplinary treatment team
Serves as an advocate and resource, providing the client with community support information, assist clients to arrange essential appointments and accompany clients to those appointments requiring attention during their stay
Responsible for conducting post-discharge follow-up for clients within 30 days of discharge
Other case management duties as directed by the clinical director or program director
Requirements for the Position
High School diploma or equivalent
Minimum Two years' professional experience
One-year experience in human services, substance use and mental health field preferred
Certification or eligibility for certification in substance abuse counseling and willingness to pursue certification preferred
Thorough knowledge and understanding of the federal regulations pertaining to confidentiality of substance abuse treatment
