Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary




The Family Intervention Specialist is responsible for providing one-on-one counseling and case management services to women who are pregnant and victims of domestic violence, that reside in the Indianapolis Healthy Start catchment area and/or the Indiana Women's Prison. The Family Intervention Specialist will interface with the Indianapolis Healthy Start program, with identification of victims and subsequent intervention to provide: education, training, and technical assistance to community agencies, service providers, and the community at-large concerning family violence - especially as it relates to infant mortality.




Essential Functions and Responsibilities




• Demonstrate Friendly Access by supporting corporate values: Community, Leadership, Excellence, Accountability and Respect (CLEAR)
• While collaborating with Indianapolis Healthy Start staff, identify pregnant women who are victims of domestic violence and recruit for services
• Assist victims of family violence in the creation and recording of a realistic safety plan
• Provide case management service at multiple sites, such as: Eskenazi Health inclusive of the Emergency Room, Outpatient Care Center, and OB/GYN clinics; shelters, Indiana Women's Prison, and Crane House.
• Participate as a public speaker to educate service providers and the community at large concerning the issues of family violence with a focus on increasing awareness of Adverse Childhood experiences and the impact on behavioral health and infant mortality, Inform clients of available resources to address medical needs, social needs, nutrition, and care coordination services for her pregnancy
• Complete all required data collection forms provided by the Division of Healthy Start & Perinatal Services (DHSPS)
• Provide training and technical support to victim advocates in central Indiana who provide services to this population
• Refer father or male partner to IHS Fatherhood Engagement Clinician
• Attend various workshops and conferences to increase awareness and understanding of victim issues and issues related ACEs and infant mortality
• Actively collaborate with law enforcement and the criminal justice system
• Provide health education including, but not limited to the following topics; prenatal care, healthy relationships and safe sleep at the Indiana's Women Prison and Crane House.
• Participate in all Healthy Start required activities as outlined in the Service Agreement Initiates social service plans for patients/clients through professional collaboration with medical, nursing and health care personnel utilizing screening questionnaires, diagnostic assessments and individualized therapeutic care plans
• Initiates advocacy/liaison role for patients/clients in referrals to and from community agencies: collaborates with agency and hospital personnel in referral activities: cultivates referral resources




Job Requirements




• Minimum of a Master's of Social Service or related field degree including but not limited to a MSW
• Active licensure as a Social Worker or Clinical Social Worker or equivalent related field.
• Social service experience in a healthcare setting dealing with maternal child health, pediatric, adolescent and/or adult




Knowledge, Skills & Abilities




• Effective communication skills
• Ability to use age appropriate communication and skills detailed in the department/area/unit job specific/age specific competencies
• Analytical, psychosocial assessment and counseling skills
• Knowledge of pertinent State/Federal laws and regulations, community systems and resources
• Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, initiative
• Ability to apply social service techniques/principles including evaluation, intervention, and case management
• Ability to perform duties independently under the direction of specified supervisor
• Ability to recognize own limitations and seek assistance from appropriate resources as needed
• Proficient writing skills for presentations, posters, reports etc.
• Ability to learn the IHS Core Services Tracking System (CSTS) for data entry
