The primary function of the MDS nurse is to assess resident care needs, direct and supervise staff to meet the resident's needs, coordinate with other members of the Inter-Disciplinary Team (IDT), develop and implement a plan of care that meets the individual needs of each resident. The MDS nurse also provides direct resident/patient care and reports to the DON. The ideal candidate possesses strong clinical judgment, excellent communication skills, and a deep understanding of long-term care regulations.

Essential Duties and Responsibilities:

This role focuses on ensuring accurate and timely completion of Minimum Data Sets (MDS) for skilled nursing residents. Key responsibilities include overseeing PDPM compliance, including analyzing data, identifying improvement areas, and providing recommendations to optimize reimbursement. The MDS Nurse will also assess residents, coordinate MDS assessments, and ensure data accuracy. This includes managing level of care changes, conducting resident care conferences, and providing training to new MDS Coordinators. Additionally, this role involves tracking and monitoring Quality Measures and ensuring adherence to all relevant regulations and maintaining resident confidentiality.

• Maintain a system by which new admissions, annual reviews, and quarterly reviews are to be initiated and completed within the required periods.

• Conduct observations and interviews as well as evaluations required for MDS and/or care plan preparations.

• Complete the Minimum Data Set (MDS) within 7 days of admission in coordination with other members of the Inter-Disciplinary Team.

• Review orders with the appropriate nurse daily or more frequently if necessary.

• Provide direct resident/patient care as needed.

• Review accidents/incidents and critically ill/unstable residents.

• Review New Admissions/Medicare Specialized therapies, i.e., IV Therapy and Pressure Sores, Stage 3 and 4.

• Review and assess resident's condition upon a reported accident/incident within 24 hours.

• Complete documentation and report to the Director of Nursing.

• Ensure all areas which are triggered on the resident Assessment Protocol Summary, are included on the resident's plan of care.

• Must always maintain employee/resident/patient confidentiality.

Qualifications:



  • Education: Graduation from an Accredited School of Nursing with a Bachelor of Science in Nursing (BSN) preferred.


  • Experience:

    • Minimum 3 years of experience in long-term care.

    • Supervisory experience preferred.




  • Licensure/Certification:

    • Current and valid Registered Nurse (RN) license in the state of employment.

    • Maintain all required continuing education and licensure requirements.




  • Skills:

    • Strong clinical judgment and assessment skills.

    • Excellent communication, interpersonal, and organizational skills.

    • Proficiency in Microsoft Office Suite, PCC, and electronic health records (EHR) systems.

    • Knowledge of PDPM, MDS, CRC, and long-term care regulations.

    • Ability to work independently and as part of a team.

    • Strong analytical and problem-solving skills.
