Job Summary

Under the direction of the Senior Manager of Behavioral Health Integrated Care Management leadership, the Peer Recovery Specialist supports members by utilizing their own lived experience. The Peer Recovery Specialist has been successful in the recovery process and assists members that are experiencing similar situations. Through shared understanding, respect, and mutual empowerment, the Peer Recovery Specialist assists members with continued engagement in the recovery process to reduce the likelihood of relapse, effectively extending the reach of treatment beyond the clinical setting into members' communities/ environments to facilitate successful, sustained recovery. The Peer Recovery Specialist encourages members to regain independence, develop skills for self-advocacy, and participate in recovery-oriented activities and treatment services as indicated. Additionally, the Peer Recovery Specialist supports members in developing recovery-oriented goals and complete their recovery plans while identifying and accessing socioeconomic determinants of health needs.
The Peer Recovery Specialist serves as a role model, mentor, advocate, and motivator to recovering members to promote sustainable, long-term recovery. The Peer Recovery Specialist addresses the needs of members diagnosed with a severe substance use disorder and is focused on providing a high-touch community presence during the transition period between acute levels of care, residential treatment and/ or returning to the community and supporting early recovery success.

Key Responsibilities/Duties - what you will be doing


  • Exercise independent decision-making based on data and member population trends to identify and engage members in Care Management.

  • Co-facilitate the ongoing development and growth of the Integrated Care Management department and overall Point32Health organization as a subject matter expert with monthly substance use disorder educational series; audience is comprised of Enterprise Care Management teams from Community Health Workers to Managers and Supervisors.

  • Build rapport and trust through shared lived experience with members during periods of transition to support adherence with treatment plan and ensure a successful return to the community; create mutuality with members in support of their stated goals and desires around recovery.

  • Provide face-to-face and/ or telephonic support, education, and advocacy for members while promoting independence and skill-building within their own recovery networks in their communities and ongoing conversation around community recovery supports and the needs of the member. This may include but is not limited to community recovery supports such as 12-Step, SMART Recovery, Recovery Dharma, and Refuge Recovery.

  • Utilize motivational interviewing, mindfulness exercises, and other peer-supported, evidence-based practices, and works with members in varying states of transition/ recovery to assist and coach members in establishing recovery-orientated, member-centered goals and completion of a recovery plan.

  • Care coordination alongside/ not limited to interdisciplinary Care Management teams comprised of Nurse Care Managers, Behavioral Health Care Managers, Community Health Workers, treatment facilities, external agencies, outpatient providers and other member supports as appropriate and indicated while upholding HIPAA standards and confidentiality of protected health information.

  • Identify and escalate high risk issues promptly and appropriately to the Senior Manager around member safety concerns.

  • Fulfil or exceed established key performance indicators as it relates to member outreach and engagement on a consistent basis, adheres to documentation and workflow standards.

  • Other duties and projects as assigned.

Qualifications - what you need to perform the job

EDUCATION, CERTIFICATION AND LICENSURE:


  • Associates degree or higher required, in Human Services or health care-related field

  • Completion of Certified Peer Specialist (CPS) certification program or successfully completing within the first six months of employment. Certification must be kept current.

EXPERIENCE (minimum years required):


  • At least 5 years of experience as a Peer Recovery Specialist, Recovery Coach, or in a similar position in a relevant environment or equivalent educational experience

  • Lived experience with recovery from a substance use disorder and willingness to self-identify as being in recovery from substance use disorder

  • Demonstrated utilization of recovery tools and skills to be shared in support of members

  • Experience with care coordination and/or care management

SKILL REQUIREMENTS:


  • Understanding of the concept of recovery as it relates to behavioral health, peer support/recovery coaching and wellness, and recovery planning

  • Excellent written and verbal communication, time management and organizational skills

  • Strong interpersonal skills: ability to foster relationships with individuals at all levels internally and externally (e.g., nurses, social workers, community health workers, leadership, etc.)

  • Bilingual highly desirable

  • Receptive to feedback

  • Self-motivated and able to work independently and set priorities

  • Experience with Motivational Interviewing and working with individuals in different Stages of Change

  • Experience with community-based visits

  • Understanding of the managed care environment with focus on the Medicaid population

  • Knowledge of community resources to address basic needs such as housing, food, transportation, clothing

  • Knowledge of barriers encountered by low-income and culturally diverse families

  • Proficiency with Microsoft Office Suite and typing

WORKING CONDITIONS AND ADDITIONAL REQUIREMENTS (include special requirements, e.g., lifting, travel):


  • Must be able to work under normal office conditions and work from home as required

  • Valid driver's license and automobile with active car insurance policy required

  • Travel using employee's own car to Massachusetts substance use facilities will constitute approximately 50% of the work on average

  • Mileage will be reimbursed per industry standard

  • Work may require simultaneous use of a telephone/headset and PC/keyboard and sitting for extended durations

  • May be required to work additional hours beyond standard work schedule
