SHIE CUSTOMER SUCCESS MANAGER (SENIOR PROGRAM SPECIALIST)

$99,507.20-$133,390.40 Annually

*Provisional Appointments: For a provisional appointment, a civil service exam is not required. However, to obtain a regular position, the appointee will need to compete successfully in a County Exam when open.

This position requires CA residency.

After satisfying a training period, an Alternative Work/hybrid schedule will be offered.

About Social Health Information Exchange Division

Alameda County Health strives to interact, understand, and promote accessible, responsive, and effective services for the people and communities of Alameda County. We must constantly evaluate and initiate new programs for the changing needs of all residents. Alameda County Health created the Social Health Information Exchange (SHIE) to securely collect and integrate medical, mental health, substance use, housing, social care, legal, incarceration, and crisis response information to understand a consumer's and a population's care needs from a "Whole Person" perspective. The SHIE serves as a community resource for data exchange, supporting Alameda County health and social services providers to improve care coordination and service delivery for their consumers. The SHIE provides data exchange services, custom cross sector data analytics, and access to provider facing applications that support coordination of care, such as the Community Health Record. The SHIE team is recruiting a Client Success Manager to support engagement efforts with a growing community of data exchange partners.

THE POSITION

The SHIE team is looking for a Client Success Manager to engage with leaders and providers from multiple sectors of care across Alameda County. This position will provide support for troubleshooting a wide variety of SHIE issues and coordinating problem solving efforts, as well as managing relationships with a portfolio of data sharing

partners. This will be an exciting and varied position and will require someone who is highly relational and

intuitive, organized and detail-oriented, yet flexible and a creative problem-solver.

DISTINGUISHING FEATURES

This role requires high relationship systems intelligence, technical acumen, and an understanding and enthusiasm for how program planning and evaluation can be made more powerful when informed by curated health and social data. This position is a business developer, thought partner, problem solver, data visualization designer, and translator between stakeholders and the broader SHIE team who can draw out the power of the SHIE's integrated cross-sector data. More importantly, this individual will help stakeholders connect the dots between the SHIE inputs and outputs to ultimately derive stakeholder value, and revenue, from the SHIE.

Major Tasks, Duties and Responsibilities:

  1. Serve as the primary ongoing program contact for a portfolio of SHIE clinical and non-clinical data exchange partners and program participants.
  2. Represent project leadership at meetings and with internal and external stakeholders.
  3. Provide technical assistance and broker other necessary support for client-facing staff of partner organizations.
  4. Conduct research and analysis for troubleshooting a wide variety of issues using quality improvement methods.
  5. Capture complex concepts and put into accessible written and visual formats; use program data to write and develop narrative descriptions for trainings and presentations on a wide variety of subjects.
  6. Create professional documents for a wide range of purposes and audiences using Visio, Word, Excel, PowerPoint.
  7. Liaison between Community Health Record Onboarding Team and program as needed.
  8. Take on a variety of special projects as needed.
  9. May perform other duties as assigned.


MINIMUM QUALIFICATIONS

EDUCATION:

The equivalent to graduation from an accredited four-year college or university (180 quarter units or 120 semester units) with major coursework in business, public administration, social science or a field related to the program area to which assigned.

(Additional experience in providing direct services to clients or in program planning and evaluation may be substituted for the education on a year-for-year basis.)

AND,

EXPERIENCE:

The equivalent to two years of full-time professional level work with independently planning and evaluating client service delivery programs at a level equivalent to or higher than Program Specialist or Program/Financial Specialist in the Alameda County classified service. (Non-classified includes District Attorney's Office, Hospital Authority, and the Consolidated Courts.)
