Qualifications & Requirements:

• Have relevant education, licensure, and time in practice


• Are licensed to practice in the state of Florida


• Are Board Certified as an OCS and/or SCS


• Have practiced for at least 6 years


• Want to work in a collaborative care team environment with other physicians, mid-level providers, physical therapists, and health coaches


• Practiced physical therapy in a collaborative setting with other provider types


• Have experience with conservative outpatient spine and joint care


• Have experience with surgical aftercare


• Provided care in the home setting
