Classification: Hourly, Non-Exempt. Thirty-five-hour (35.00) week; evenings and weekends expected for meetings, workshops, and seminars.
Position Summary: The Q Center Social Support Specialist is responsible for delivering both individual and group services. In group settings, this role involves providing technical assistance and education to schools, providers, and other organizations/agencies, as well as supporting school-based GSA programs and advisors in Central NY and the Mohawk Valley. The Support Specialist will offer one-on-one experiences through linkage and navigation services for young adults aged 18-29, managing a caseload of 15-20 clients across the two regions. Additionally, this position includes assisting with drop-in activities, outreach, and community events as needed.

Minimum Qualifications:
At least two (2) years' experience providing stigma-free, non-judgmental and culturally responsive services to diverse populations.
Access to a reliable automobile that meets DMV requirements (insurance/registration). A valid NYS driver's license is required for necessary travel.
Ability to work daytime and evening hours Monday through Friday, with occasional weekend hours.

Primary Responsibilities:
Develop and maintain partnerships with school-based LGBTQ+ student groups and advisors, including the provision of educational opportunities, technical assistance and education prioritizing creating safe and supportive schools in Central NY and Mohawk Valley.
Assist in the facilitation of activities for LGBTQ+ youth and young adults ages 13-29 designed to increase self-efficacy and independent living skills; reduce social isolation and increase social supports; increase awareness of LGBTQ-affirming providers; promote health and wellness utilizing a risk-reduction/harm-reduction/sex positive approach; and cultivate pride and leadership skills.
Encourage participation in program activities and form healthy relationships by being a positive adult role model and mentor,
Complete necessary paperwork including care management intakes and progress Notes Ride Logs, and Incident Reports as required. Maintain accurate client and service records in accordance with the Department's policy, procedures, confidentiality, and compliance.
Prepare all required monthly and program reports as assigned, and ensure thorough, up to date and complete client records.
Conduct ongoing assessments with clients, as required by funders, and report findings to LGBTQ Program Manager on a quarterly basis.
Provide crisis intervention for youth and families and make referrals to internal and external programs/services as needed.
Assist LGBTQ Program Manager in educational content creation for Q Center social networking sites.
Assist in scheduling and facilitating LGBTQ+ cultural responsiveness and competence workshops, and report results of workshop evaluation.
Perform community outreach to increase awareness of Q Center programs and activities.
Participate in department and agency case conferences, including Grand Rounds.
Complete/submit all required documentation within designated time frames.
Assist with the general work of the Family, Education, and Support Services Department including special projects and general agency programs/initiatives.
Participate in appropriate and applicable training/professional development opportunities for a minimum of 24 hours of curriculum-based training annually.
Participate as a culturally competent member of ACR Health, which includes having the capacity to function effectively as an individual within the context of cultural beliefs, behaviors, and needs presented by clients and their communities.
Work in a cooperative, collegial style with all other organizational personnel. Conduct oneself in a professional manner as an agency employee and representative.
Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F and HIPAA, ACR Health policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols. Observe and abide by the HIV Confidentiality Law and HIPAA.
Perform other duties as assigned by Director of Youth & Family Services, Chief Program Officer and/or Chief Executive Officer.
