We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Team Nurse assumes responsibility for nurse visits at Amoskeag Health. They triage all telephone calls and walk-in patients with medical questions/concerns. The Team Nurse position also coordinates the flow of patient communications regarding such things as labs, refills, dosages, changes, and instructions from providers.
