Duration: Long-term
Schedule: 5 hours per week.
Location: Remote. Must be Vermont resident.
Non-exempt Hourly Rate: $73.50
Core Responsibilities:
The primary purpose of this position is to develop, as a member of a multidisciplinary team, a psychosocial assessment and treatment plan for each patient with a specific focus on successful community reintegration and incorporating prior community functioning, forensic encumbrances, inpatient and outpatient treatment needs, family involvement and patient preferences and goals.
Clinical Services
Conducts diagnostic interviews, observations and contacts the community treatment providers, forensic coordinator, probation officer, entitlement providers and family members and/or significant others.
Performs evaluation of admission materials and obtains data essential for treatment planning and documents same in a full psychosocial admission assessment according to current department guidelines.
Collaborates with treatment team members to determine whether further diagnostic studies or information are needed.
Participates in developing individual Master Treatment Plans with treatment team members and take the lead in development of a transition and discharge plan.
Coordinates the completion of all applications for discharge including housing, financial, work, and psychosocial outpatient programs.
Ensures that legal encumbrances are adequate addressed and insures timely follow up with probation and parole officers.
Attends court hearings at Circuit, District, and Family courts as needed on all islands.
Provides individual and group psychotherapy, counseling, supportive therapy, training, or other modalities of treatment in compliance with the patient's Master Treatment Plan.
Establishes and maintains a therapeutic relationship with every patient assigned to the inter-disciplinary team.
Coordinates inter-agency staff reports for patients considered appropriate for discharge.
Prepares and maintains statistical forms and reports related to patient admission, changes of legal status and discharge and documents same in patient’s medical record.
Plans and coordinates appropriate discharge and aftercare for each patient on the assigned caseload and documents the plan throughout hospital episode and on the Discharge Summary.
Ensures that proper education and counseling to family, caretakers, and/or other persons significant to the patient's treatment team are provided, and documents same in-patient record.
Community and Inter-Agency Relationship
Initiates case conferences with collaborating statewide agencies to coordinate diagnostic evaluations, treatment services, written referrals to appropriate agencies and discharge planning.
Represents patients and families in community planning meetings with public or private service providers and agencies.
Visits collaborating agencies, community case management providers and similar organizations to coordinate diagnostic evaluations, treatment services and discharge planning for individual cases.
Plans and coordinates the patient's community reintegration with staff of community case management providers. This includes community visits, living arrangements, financial assistance, medical/psychiatric case management, adult probation follow up and legal guardianship.
Implements discharge and after care plans for patients by attending Adult Mental Health Division meetings, visiting placement sites, clinics, Department of Social Services and Housing facilities, Honolulu International Airport, and the like.
