Under the direction of Home Hospital Leadership, the Clinical Manager for Nurse Navigation, will provide administrative, clinical, and operational management, oversight and leadership to support activities identified as key to success for our program.
The Clinical Program Manager for Home Hospital Nurse Navigation will lead efforts to support our system's patient identification and admission processes. This position will be responsible for co-designing, implementing and leading clinical and operational initiatives focused on helping providers and care teams accurately identify patients for Home Hospital admissions. This position will collaborate closely with MGB stakeholders, to establish a foundational program infrastructure and build. This position reports to the Director of Case Management.
Principal Duties and Responsibilities
Lead oversight of the Nurse Navigation process and evaluation
• Provide leadership and management oversight of the HH Nurse Navigator team. Work to support the ongoing development of the Nurse Navigator role, in partnership with HH Leadership team, create educational criteria and training build in conjunction with Home Hospital education team, engage in process improvement and strategy for the overall design, implementation and evaluation with Home Hospital leadership, project management, education and facility-based stakeholders.
• Partner with Home Hospital Clinical leadership on program strategy and operations.
• Participate in, and accept ownership for, cross-department initiatives
• Anticipate, identify, and drive resolution of program challenges to prevent downstream problems
• Oversee and collaborate with Home Hospital team on project related deliverables including project plans, PowerPoint presentations, status reports,
• Independently develop material and communicate updates, findings, and recommendations to internal program steering committees, network committees and audiences external to MGB Home Hospital.
• Contribute relevant information to leadership for budget development
